Important Instructions

Below are general instructions to be followed while filling the AMP Membership Form
1) Form should be filled in English only.
2) You need to send an online copy of this form to us via email on info(at)ampindia.org 3) You will be required to paste a passport size photograph on the form. The photograph should be original, and not a photocopy. Please stick the photo, do not staple.
4) You need to send the Hard Copy of your membership form on following address. Without which membership to AMP will not be considered. Address: 68/69 Umar Manzil, 5th Road, khar (W), Mumbai 52, Landmark Opp. BMC Market.
5) The Initial Membership in the AMP is Rs.500/-. Please revert to us if you can not pay Initial Membership fees.
6) The Monthly Membership in the AMP is Rs.200/-Please revert to us if you can not pay Monthly Membership fees.
7) Students are expemted from any fees.

Documents to be furnished
1) One age proof document
2) One address proof document.

1) Birth Certificate
2) Passport
3) PAN Card
4) Driving License
5) State Board Exam Certificate or School Leaving Certificate
6) Certificate granted by medical practitioner not below rank of Civil Surgeon
7) Sworn Affidavit before Executive Magistrate or First Class Magistrate or Notary Public
8) Panchayat Secretary or Village Administrative Officer/Taluk Certificate
9) EPIC Voter ID
10) Ration Card (in name of Member)
Note: Either ID/ Address or Age proof must have a photograph on it

List of Projects:
1 Career Counselling and Mentorship Program
        This project deals with providing an online career counselling to students through telephone or via Voice Chat, Emails. This will help students deciding and choosing a best career option available after 10th, 12th or Graduation.
In the Mentorship Program you will have one-to-one relationship between few students till they graduate or before. As a Mentor you will have to provide them consistent support & guidance to help them complete their education & shape a good career

2 Skill Devlopment Workshop
        This skill development programs are conducted every weekends in majority of the areas identified by AMP members. Those who are part of this project can bring prominent speakers to these workshop and be the part of drive to create awareness among Muslims regarding these workshops. You can be a part of this activity by nominating your name to be the Coordinator for these workshops in your locality

3 Scholarship Fund
        This project deals with collecting funds for scholarships which will be provided to needy students for higher studies. The scholarship provided to students will be interest free and students have to return the money after a certain period of time after their degree completion.

4 Research Team
        This team will be responsible to do research in various areas decided by the Research Committee which will basically deal with Muslim issues. Research work will be send for publicatioin to various news papers and magazines.

5 Membership Drive
        Members of this group will be responsible to be the part of membership drive and they will work on various plans to increase the membership of AMP.

6 Organization Support
   Members of this group will be responsible to be the part of membership drive and they will work on various plans to increase the membership of AMP.
